Description (Service)

Intensive Services provide services to children, youth who have been identified with having mental health needs that require intensive intervention to maintain their functioning at home, school, and/or in the community. Services are developed in partnership with the family to make sure they receive what they need and when (e.g., home, community, school or core service procedure setting). The goal of Intensive Services is to: reduce the severity of mental health problems, strengthen coping strategies; increase awareness and understanding of the problem; and improve functioning. Intensive Services include: counselling in a comfortable environment; parenting strategies designed to promote positive family functioning; individual and/or family therapy (for the entire family as identified by parents); hours of support are determined upon assessment at the time of entry (times may vary); service collaboration and case management; strong emphasis on community partnership working together; and linking families to community resources/support

Goals of Intensive Services are to:

Wraparound Brant: Wraparound services are a specialized service available to support children and youth and their families with complex needs. Complex needs are defined as having concerns in more than one area (e.g. mental health and developmental services) and to avoid family breakdown. A Wraparound facilitator identities the family's strengths and needs, and develops a personalized plan to address those needs. That plan will include the following services: In-home counselling, school support and/or community support (e.g., recreational activities); Individual and/or family therapy (for entire family as identified by parents); Behaviour management programming and parenting strategies; Service co-ordination by your Wraparound Facilitator; A family meeting with informal supports, school and community partners to review the treatment plan. Revisions to the plan will be made as necessary; Respite services providing time limited relief for the family.

The Wraparound Service model includes partnerships, coordination of services, family support, individualized support, flexibility and ensuring services are family-centered

That plan will include the following services:

  • in-home counselling

The Early Years Program is a home, school and community-based program that promotes healthy social and emotional development of children from birth to 6 years of age and their families. Services are customized to meet the individual needs of each child and family, matching the level of need with appropriate intensity of service. Hours of service are flexible and include both evenings and weekends.

The Early Years Program also provides consultation, support and training services to community programs that are involved with children from birth to six years of age
