Description (Service) Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services 
Supports for people with Alzheimer's disease and other dementias, and their caregivers 
Seminars, workshops and training sessions * one-on-one care plan consultations with health professionals * advanced care planning for future health care needs 
Education Programs and Resources - Education, awareness programs, and resources for the general public and health care providers on Alzheimer's disease 
Safely Home Alzheimer Registry - National registry that help people with dementia to return home safely if they become lost * information will be shared with the Royal Canadian Mounted Police only for locating a missing person * national website  
One-on-one counselling and group supports for caregivers, including family and friends 
Peer support and discussion for caregivers, including family and friends * educational video series 
Peer support groups, education, and training for family members, service providers, and caregivers * sessions may include information such as advanced directives and safety issues 
First Link Referral Program - designed to help newly diagnosed individuals with dementia get the help they need as soon as possible 
Behavioural Supports Ontario Community Outreach Team & Intensive Case Managers help older persons, who have a cognitive impairment and are experiencing challenging behaviours, continue to live in their homes 
Meaningful Volunteer Opportunites that offer learning experiences and enrich our programs and services
