• children/youth 0-18 years of age with long-term physical, developmental and/or communication disabilities
• children/youth who require assistance to perform physical and/or functional tasks in a manner similar to that of a non-disabled person of the same age
• reside in Brantford or Brant County with parent or legal guardian
