Working with community organizations (education, health, and children's services) Service Coordination Brant offers integrated service planning to families. Service coordination ensures that there is a clearly identified staff for each family to lead their service plan and support services working together to meet each families' unique priorities and needs

A Service Coordinator will:

• be the 'key helper' for the family
• develop and coordinate a service plan, with the family as well as health, education, and children's services
• provide support and information that fits the individual needs of the family

A service plan outlines who is doing what, and clearly identifies each family's contact staff. It is driven by what each family identifies as their main concerns and this guides the goals of each service being provided
