New Credit and other First Nations - medical transportation requires prior approval. Requests must be made at least 2 weeks prior to the appointment

Information required:

• full name
• date of birth
• complete address and telephone number
• 10-digit status card registration number
• doctor's name and the area of specialty
• date and time of the appointment
• address of appointment
• any other medical transportation needs (i.e. wheelchair, escort)

Medical Transportation to and from medical appointments covered by OHIP and/or Non-Insured Health Benefits
