Description (Service)

A personalized stroke recovery program that helps survivors and their families navigate the path forward after stroke

  • program aims to bridge healthcare gaps through system navigation, support a seamless transition from hospital to home, and help those impacted by stroke find what they need throughout their recovery and improve their quality of life
  • participants are connected with trained After Stroke Coordinators offering personalized guidance and system navigation during the recovery process 
  • in-person and virtual support available
