Description

Description (Service)

Organization promotes community-based services responsible to the needs of those with mental health concerns, their family members, and the general community

Services offered include:

  • Community Support Case Management Program - provides intensive case management supports to people with serious and persistent mental illness
  • Specialty Case Management - including concurrent disorders (mental health and addictions), complex needs (dual diagnosis / acquired brain injury), urgent needs (needing immediate, non-crisis supports), and Transition (referrals/transition from hospital)
  • Counselling Program
  • Court Support Case Management & Diversion Program - assist individuals with a mental health issue who come into contact with the criminal justice system. Provided in partnership with the office of the Crown Attorney and provide assistance with mental health court diversion, community-based case management, as well as consultation and information
  • Crisis Stabilization Bed Program - provides 6 beds in a safe and secure environment for individuals coping with an acute mental health crisis. The focus is diversion from hospital and stabilization in the community and the stay can be up to 7 days
  • Family Support
  • Families Caring, Families Sharing support group
  • Mental Health Promotion & Education Program
  • Peer Support
  • Supported Housing - affordable housing to low-income individuals. Monthly rent is geared to the tenant's income. Transitional beds available to allow residents temporary housing while they proceed on a gradual transition to independent living. There is a waiting list for housing
  • Survivors of Suicide Loss Bereavement support group
  • Vocational Support Services
