To register with Easter Seals Ontario you must:

• complete and sign the Easter Seals Ontario Child Registration Application
• have your child’s Occupational Therapist (OT) or Physiotherapist (PT) complete Section Four of the application
• email, fax or mail the completed application form to:

Provides programs and services to children and youth with physical disabilities across Ontario to help them achieve greater independence, accessibility, and integration

• assistance with the cost of incontinence supplies, assistive and adaptive technology
• summer camp programs
• Access 2 Card - card that allows a support person accompanying someone with a permanent disability, free access to participating movie theatres, cultural attractions, entertainment venues and recreational facilities in Canada
