Individuals and families who are living with dementia, mild cognitive impairment, or are at the greatest risk of developing dementia

Call or email if concerned about memory loss or dementia diagnosis - no referral is needed  
 
Health Care Professionals can refer patients/clients to programs/services or request the support of a psychogeriatric resource consultant or Behaviour Supports Ontario worker. Physicians may request cognitive testing for patients who reside in Haldimand Norfolk. Referral forms are on the website: www.alzda.ca, under the Health Care Professionals tab

The HAC (Health, Activity, Community) is a community-based service of the Alzheimer Society, bringing advanced health, wellness, activity, and connection to locations throughout Haldimand County

Programs are designed to help individuals stay active, build new skills, and engage with others in a welcoming and supportive environment. Offering focused support for people living with dementia, acquired brain injuries or brain changes, and vision challenges, using approaches that reflect the latest best practices in community wellness

To ensure accessibility for everyone, programs are offered both in-person and online providing flexible options that meet the diverse needs of each client
