Description (Service) Information and support for individuals with Alzheimer's disease and other dementias, and their carers at the time of diagnosis and throughout the course of the disease. Persons with memory loss and their families can access education and support services 
  • Health Promotion Activities - opportunities to connect with others in similar situations and to engage through meaningful activities to ‘live well’ 
  • Education Programs and Resources - education, awareness programs, and resources for the general public and health care providers on Alzheimer's disease 
  • Medic Alert Safely Home - relay your key information to emergency responders - register at this website: 
  • One-on-one counselling and group support - for persons diagnosed with dementia as well as for family and friends  
  • First Link Referral Program - designed to help newly diagnosed individuals with dementia get the help they need as soon as possible 
  • Psychogeriatric Resource Consultant - provide support to health care professionals who care for seniors with complex needs that have associated behaviours 
  • Behavioural Supports Ontario - Community Outreach Team help older adults, who have cognitive impairment and are experiencing challenging behaviours, continue to live in their homes 
  • Meaningful Volunteer Opportunities - offer learning experiences and enrich the programs and services 
  • Specialized programs - groups for persons diagnosed with Mild Cognitive Impairment and self-care for caregivers
