  • assessment will include physical tests and an interview about personal needs and day-to-day activities

In-home Occupational Therapy services:

  • home safety assessment; bathroom assessment; ergonomic assessment
  • mobility assessments to apply for the Assistive Devices Program for walkers, wheelchair and power wheelchair. ADP authorized to complete applications
  • transfer assessment and training for staff
  • equipment prescriptions 
  • navigation of government funding
  • falls prevention
  • home accessibility
  • Independent Living Skills Program - passport funding

The Occupational Therapy Assessment fee includes:

  • an initial interview
  • physical, cognitive and functional assessment
  • equipment recommendations
  • trial of equipment
  • follow up
  • complete and submission of required paperwork (ADP and March of Dimes)
