Application	Call to schedule an appointment an Occupational Therapist will provide an in-home mobility assessment assessment will include physical tests and an interview about personal needs and day-to-day activities

In-home Occupational Therapy services:

home safety assessment
mobility assessments to apply for the Assistive Devices Program for walkers, wheelchair and power wheelchair. ADP authorized to complete applications
transfer assessment and training for staff
equipment prescriptions
navigation of government funding
falls prevention
home accessibility
Independent Living Skills Program - passport funding
